What is Home Based Care
Home Based Carers provide supervised care and support to patients and their immediate families/household members in the home and appropriate community-based settings in order to promote, restore and maintain a person’s maximum level of comfort, function and health including care towards a dignified death.

Some Duties of Home Based Carer:

Assess, plan and provide holistic (physical, spiritual, emotional and social) care and support for patients and household members

Assist clients with personal care (dressing, mobility, administering medication, personal hygiene, eating and transporting to doctor’s appointments)

Capacitate patient and household/family members in caring for the patient.

Maintain open communication between families and health care professionals regarding client’s medical and emotional condition

Make appropriate timely referrals in order to facilitate a continuum of care

Maintain records and reports regarding care and support provided (blood pressure, temperature, pulse etc.)

Document and report any changes in client’s health status
-
Ensure the highest care, safety and well-being of the client

Learners write national exams upon completion of the course, Exams are written in March and October / November every year.
Certification
Learners who have passed National Examination will be awarded Occupational Certificate: Home Based Care by Quality Council for Trades & Occupations (QCTO).
